Discharge of a Patient
Procedure
- Confirm the written discharge order and that all treatment is complete.
- Take and record a final set of observations.
- Remove any remaining cannula, catheter or drain and dispose of them safely.
- Review discharge medication with the patient and give the take-home supply.
- Explain wound care, activity, diet and warning signs that need a return to hospital.
- Give the review appointment date, card and any referral letters.
- Return valuables and property, and have the patient sign for them.
- Settle the bill or NHIS claim and confirm transport home is arranged.
- Complete the discharge summary, nursing notes, bed state and ward register, then strip and disinfect the bed.
