Checking and Recording of Pulse
Procedure
- Explain the procedure and let the patient rest for five minutes.
- Wash hands and position the arm comfortably and supported.
- Place the pads of your index and middle fingers over the radial artery — never your thumb.
- Press gently until you feel the pulsation clearly.
- Count for a full 60 seconds; count for a full minute in every irregular pulse.
- Note the rate, rhythm, volume and the condition of the arterial wall.
- Compare with previous readings and with the apex beat if the rhythm is irregular.
- Wash hands and record the rate and character on the chart.
- Report any rate below 60 or above 100 in an adult, or any new irregularity.
